Registration
If you wish to register as a patient with the Practice, please check that you are living within our practice boundary. If you are unsure, the receptionist can help you.

If you are living within our area the receptionist will ask you to complete a “registration form” and a “new patient questionnaire”. If you have a preferred GP, you can indicate this on the form. However this does not affect your rights to see any of the doctors at the practice. There is a 24 hour administrative period, after which time you may use our services..

Please remember to check-in at reception when you attend for your appointment. You can use the touch-screen automated check-in system located just beyond the reception counter – please ask the receptionist to help you if you are not sure how to use it.

If you are checking in for an appointment with the Nurse, the waiting room referred to on the screen is round at the Treatment Room (not in the main waiting area).

Change of personal details

If you change your name or address or telephone number please notify the surgery as soon as possible. A change of address may mean that you are living outside the practice boundary and we may need to ask you to register with a practice elsewhere. The receptionist will be able to advise you if the new address is within the boundary. It is your responsibility to inform any hospital where appointments might be pending, of your new details.
